My child listed in this form has permission to participate in concerts, rehearsals, tours, or other events sponsored by FHMA. I acknowledge FHMA is not a drop off location and that students are responsible for travel arrangements to and from all activity locations and parents and/or legal guardians are responsible for their supervision and safety at all times.

I understand that some of these activities are held at locations such as fairgrounds, adult day health centers, schools, churches, parade routes, shopping centers and similar locations where there may be hazards associated with, but not limited to, vehicular traffic, obstacles, heavy or sharp objects, slippery surfaces and injury from contact with other individuals.

In the event of any injury to my child, I hereby discharge, release and hold FHMA and its members, agents, officers and employees harmless from liability for damage or claims for damage for personal injury, including accidental death, as well as from claims for property damage which may be incurred by the undersigned and/or the child listed in this form.

As the undersigned parent or guardian I hereby assume all risk of personal injury, sickness, death, damage and expense as a result of this participant’s participation in all recurring and non-recurring concerts, practices, rehearsals, tours or other events. I understand that participation in FHMA and its concerts, tours, and activities is voluntary and at our election and choice, FHMA is a not-for-profit corporation and that its board of directors are volunteers and serve without compensation.

I, the undersigned, being the parent or legal guardian of the minor named above, hereby grant FHMA and its affiliates, agents, and representatives the irrevocable right to use the photographs, video recordings, and/or audio recordings of my child in any for promotional purposes, including but not limited to: Promotion, marketing, and advertising materials, websites and social media platforms, print publications or digital media.

This permission includes the use of my child’s image, likeness, and voice, as part of the Materials. I understand that I and my child will not receive compensation for the use of these Materials, and I waive any claims to such compensation.

I acknowledge that FHMA will be the sole owner of all rights to the Materials and may use them indefinitely, without any further consent from me or my child.

I hereby release FHMA, its members, officers, employees, agents, and affiliates from any liability arising out of the use of the Materials, including any claims for defamation, invasion of privacy, or violation of any right of publicity.

Foothill Homeschool Musical Arts Classes

Below is a description of the classes that we offer at our FHMA sites. Please check site schedules for class availability.

Music Explorers - This 30 minute class is designed especially for ages 3-4 and allows young students
to experience music through movement, song, and games. Throughout the class, students will learn
music without realizing they are learning.

Music Fundamentals - Made just for ages 5-7, this class provides fun, easy and educational experiences to introduce children to the world of music. Simple theory, rhythm, and music history are taught as students begin to read music and explore a variety of instruments in a small class environment. Classes are 45 minutes long.

Elementary Choir - This class allows students to learn music through song and story. Throughout the year, students will learn how to match pitch, sing basic rhythms, and recognize musical terms & signs. Students will also receive training in basic vocal technique and sight singing. (Ages 5-10)

Youth Choir - Students will be taught basic sight singing, basic & complex rhythms, and recognition of music terms & signs. Students will sing various types of music throughout the year and learn how to sing 2 and 3 part harmony. (Ages 11+)

Beginning Strings - Students will learn posture, technique, and ear training on violin, viola and cello. They will develop skills by playing short melodies and folk songs. (Ages 9+)

Intermediate Strings - Students expand upon string technique by learning folk songs and classical melodies and harmonies on violin, viola, and cello. Students perform in a small ensemble and individually at the end of semester concerts and recitals. (Instructor Consent Required to join class)

Concert Band - Concert Band is designed for students who have little or no prior musical
experience. The instructor will help students choose an instrument best suited to them. Students are taught to read music and play while listening and blending with the others in the band. (Ages 10+)

Beginning Keyboard I - This is a beginner class for students ages 5-7 who are brand new to piano. During the first semester, students learn basic theory & technique. During the second semester, students learn basic note reading. Parents are encouraged to sit in class with students younger than 7. Classes are 45 minutes long. (Ages 5-7)

Beginning Keyboard II - This is a beginner class for students ages 8+ and for students who have taken Keyboard 1. Students learn basic theory & techniques, and sightreading. (Ages 8+)

Intermediate Keyboard - This class is for students who have taken Keyboard 2 or for students who have taken a year or two of private lessons and are somewhat comfortable with the piano. Students will be introduced to upper level theory and will be further challenged in their technical skills. (Instructor Consent Required to join class, Students must be proficient sight readers)

Beginning Guitar - This group guitar class teaches students proper technique, sight reading, and music theory. Students are taught to read notes as well as to play in an ensemble. (Ages 7+)

Intermediate Guitar - This class is for students who have taken Guitar 1 or for students who have taken at least a year of private lessons and are somewhat comfortable with the guitar. This group
guitar class teaches students proper technique, sight reading, and music theory. (Instructor Consent Required to join class)
